When looking in the mirror, some people find their attention drawn to their rounded shoulder lines rather than their face. This curve becomes even more noticeable when they tie their hair up or wear clothing that reveals the collarbones.

Narrow or inwardly rounded shoulders can make the upper body appear more prominent, shorten the appearance of the neck line, and even change the overall impression of the face. As straight, well-defined shoulder lines have become a recognized aesthetic preference, more people are experiencing a loss of confidence due to shoulder contours that are difficult to change through exercise alone.

Amid this trend, “shoulder fillers,” a non-surgical body contouring approach, have recently gained attention. Particularly for individuals with naturally narrow or flat shoulder shapes, this treatment can help enhance overall body proportions, leading to growing interest in the procedure.

The forehead is the widest plane of the face and plays a key role in shaping both the frontal and profile appearance. Many patients come to the clinic saying, “My forehead is flat, so my face looks too strong,” or “My cheekbones are prominent, but my forehead is hollow, so my face lacks dimension.” In fact, the shape of the forehead plays an important role in determining the overall facial contour, perceived femininity or masculinity, and even the impression of age.

Anatomically, the forehead consists of the skin, a thin layer of subcutaneous fat, and muscle overlying the frontal bone, with the frontalis muscle covering the area. As skin elasticity decreases and the subcutaneous fat layer becomes thinner over time, the forehead gradually becomes flatter and develops hollowing.

When the forehead is naturally flat or slopes backward due to its skeletal structure, the profile line between the forehead and the nose may appear straight or concave, creating a stronger and firmer overall facial impression.

I do not consider forehead filler to be simply a procedure for adding volume. Because the forehead is a key structure that influences facial proportions, three-dimensional contour, and a soft, feminine appearance, both the aesthetic design and the anatomical approach must be carefully planned.

Before treatment, I evaluate the forehead by analyzing its vertical and horizontal curvature. From the front, a smooth, naturally rounded oval contour is considered ideal, while from the side, a harmonious S-shaped curve connecting the forehead, glabella, and nose is important. In particular, the depth of the glabella in relation to the forehead projection is a key factor in determining both the amount of filler and the injection points.

During the procedure, I use a cannula to inject filler into the supraperiosteal layer, controlling the direction and extent of vertical and horizontal distribution along the natural curvature of the forehead. I prefer fillers with high viscoelasticity, good moldability, and excellent tissue integration. When necessary, I combine injections at different tissue layers to address both superficial volume deficiency and deeper structural support.

The forehead is a highly vascular area, particularly because the supratrochlear artery and supraorbital artery run through this region, making strict adherence to anatomical safety zones essential. From an aesthetic perspective, my goal is not simply to create a more prominent forehead.

Instead, I aim to create a naturally flowing contour that is harmonious and provides three-dimensional definition from the side while maintaining a soft overall appearance.

Although forehead filler alone can significantly enhance facial dimension, failing to consider the balance with the glabella, nasal bridge, and jawline may result in an unnatural or disproportionate appearance.

Ultimately, I believe forehead filler is not simply about filling one part of the face, but about designing the overall facial impression and completing facial harmony. Only when careful design, anatomical understanding, and appropriate product selection come together can we achieve a result that is subtle yet clearly noticeable.
